Caffeine, Energy Drinks, and ADHD: Help or Harm?

Caffeine can make a child feel more alert, but it has not been shown to treat ADHD. Energy drinks add avoidable sleep, heart, and poisoning risks.

Caffeine can make your child feel more awake. It has not been shown to treat the core symptoms of ADHD, such as attention, activity level, or impulse control. Blinded child studies found no meaningful average benefit. Feeling more alert is not the same as better ADHD control, and afternoon or evening caffeine can make sleep worse. Energy drinks, caffeine powders, pills, and pouches add real risks to sleep and the heart, and concentrated caffeine can cause poisoning.

Why This Matters Now

Energy drinks are easy to buy and heavily marketed to teens. Many teens with ADHD reach for caffeine to stay awake or to focus during a long school day. Your teen may tell you an energy drink helps.

On August 11, 2025, the American Academy of Pediatrics updated its guidance for parents. It says avoiding caffeine is the best choice for children, and that energy drinks do not belong in a child’s or teen’s diet. [5] That is general-health advice. It is not proof that one sip causes harm.

Parents ask a fair question. Is caffeine a cheap way to help ADHD, or a hidden problem? This article lays out the current evidence so you can talk with your child calmly and bring good questions to the prescriber. A calm talk is more useful than a lecture.

One thing this article does not ask is whether caffeine causes ADHD. A link between a drink and symptoms does not prove a cause.

Why feeling awake is not the same as ADHD control

Caffeine reduces sleepiness. Through the day, a sleep signal called adenosine builds up in the brain, a little like sand filling an hourglass. The more it builds, the sleepier your child feels. Caffeine does not empty the hourglass. It blocks the brain from reading the level, so tiredness fades for a while. The sleep pressure is still there.

ADHD medicines work in a different way. They have been tested for symptoms and for daily function. Caffeine is not a weaker version of a prescription stimulant.

A sleepy teen may finish more homework after a caffeinated drink. That can be real. It may reflect wakefulness, not ADHD treatment. Delayed sleep can then lead to more tiredness the next day, and another drink.

What the strongest child studies found

A 2023 review found seven randomized crossover studies in 104 children ages 5 to 15. A randomized study uses chance to assign treatments. In a crossover, each child tries more than one study drink at different times. Four of these studies, with 76 children in all, could be combined. They found no meaningful difference between caffeine and placebo for overall ADHD symptoms. A placebo looks like the test drink but does not contain the tested ingredient. [1]

These studies were blinded. That means the people rating the children were less likely to know which drink each child had. Blinding limits the effect of hope and guesswork.

The trials were small and mostly old, so they could miss a small effect. Even so, no dependable treatment effect has appeared. One newer study tested caffeine, with or without an amino acid called L-theanine, in five boys. Caffeine made one stop-signal task worse. A few combination results improved, but the study ran many tests and was far too small to guide care. [2]

Why “more productive” can be misleading

ADHD care looks at real life at home and school. If your child seems more productive after caffeine, that is worth noticing, but it is not the whole picture. Also check sleep, teacher feedback, mood, and how long the change lasts. Alertness tests and symptom ratings can point in different directions, because staying awake and treating ADHD are not the same thing.

What caffeine can do to your child’s sleep

Sleep is the clearest everyday concern. In one study, 98 teens wore a home brain-wave sleep monitor for seven nights and kept caffeine diaries. More caffeine was linked with taking longer to fall asleep, less total sleep, lower sleep quality, and less dream sleep. Afternoon and evening use drove the findings. [3]

This study cannot prove that each drink caused each change. But the sleep measure was objective, not just a guess. And the timing matters. Later in the day is when caffeine seems to cost the most sleep.

Poor sleep can look like worse ADHD. A short-slept child may be restless, forgetful, emotional, or slow the next morning. More caffeine can hide that tiredness without fixing it, which can start the cycle again.

Why energy drinks and concentrated caffeine need extra care

Energy drinks usually contain caffeine, and some contain other stimulants too. Sports drinks are sold for fluid and salts, though some products blur the line. The safest habit is to read the full label every time.

Read the whole label, then add it up

On its page checked July 28, 2026, the FDA reports that a 12-ounce energy drink may contain about 41 to 246 milligrams of caffeine. [4] Added caffeine has to appear in the ingredient list, but printing the exact amount is often voluntary. So two cans that look alike can hold very different doses.

Caffeine also stacks. Coffee, soda, energy drinks, chocolate, pre-workout powders, and some medicines all add to the total. Guarana adds caffeine too, even when the word “caffeine” is not front and center. When you add up a full day, the number can climb higher than a parent expects.

Blood pressure, heart, and poisoning

In a small randomized crossover study, 27 healthy children and teens drank either an energy drink or a matched placebo. The energy drink raised the top blood-pressure number by up to about 5 points and the bottom number by up to about 3 points for several hours. [6] This short test cannot show long-term effects, or effects in a child who already has heart disease.

US poison centers recorded 32,482 single-product caffeine energy exposures in people younger than 20 from 2011 through 2023. Most reports did not lead to treatment at a health-care facility, but some serious outcomes did happen. Young children often got into a product by accident. Teens made up most of the serious cases. These are voluntary reports, so they cannot tell us the risk per user. [7]

Pure and highly concentrated caffeine is the highest-risk form. The FDA advises consumers to avoid bulk caffeine powders and liquids. A single teaspoon of pure powder can hold about as much caffeine as 28 cups of coffee, and deaths have occurred. [8] Lock powders, pills, pouches, shots, and pre-workout products away from children. Never measure bulk caffeine with a kitchen spoon.

Does using energy drinks cause ADHD?

No. Some surveys find more hyperactivity or inattention among young people who use energy drinks. [12] A small follow-up study found links running in both directions. Early energy drink use predicted rising symptom scores, and early hyperactivity predicted more coffee use later. [13]

Poor sleep, impulsivity, stress, other substances, and family or school factors may help explain the pattern. Energy drink use is a useful safety clue. It is not proof that the drink caused ADHD. A link is not a cause.

What to know if your child takes ADHD medication

No modern child trial tells us whether adding caffeine to a prescription stimulant is safe or helpful. The only controlled combination study in children enrolled six children back in 1981. [9] Caffeine and stimulant medicines can each affect sleep, appetite, blood pressure, anxiety, or heartbeat. Combining them without direct evidence calls for care. Give the prescriber the exact product, amount, and timing.

One medicine deserves a special note. Viloxazine (brand name Qelbree) can slow how fast the body clears caffeine. An industry-funded adult study found that caffeine levels rose severalfold, though children were not studied. [10] The U.S. Qelbree label current on July 28, 2026, says it must not be used with sensitive CYP1A2 substrates. [11] Caffeine is a common CYP1A2 substrate, so this caution applies to it. If your child takes viloxazine, ask the prescriber or pharmacist to review every caffeine source.

Do not use caffeine to replace or change ADHD medicine, and do not change the prescription on your own. Bring your questions and your child’s symptoms to the prescriber.

Risks, Limitations, and Uncertainties

The honest summary is that the evidence here is thin in places. The studies showing no ADHD benefit were small and mostly old, so they could miss a small effect. We still need larger child trials that use teacher, clinician, school, and objective outcomes. We need long-term studies of energy drinks. We need direct data on caffeine combined with each specific ADHD medicine. And no group of children has been shown to be a reliable “caffeine responder.”

Certainty is not the same across every claim. It is only low certainty that caffeine gives no meaningful ADHD benefit, because the trials were small. It is moderate certainty that later-in-the-day caffeine harms teen sleep. It is high certainty that pure or highly concentrated caffeine can cause severe poisoning.

On safety, watch for repeated sleep trouble, faint feelings, chest pain, an unusual heartbeat, vomiting, severe anxiety, tremor, or a sharp behavior change after caffeine. Call the clinician if these show up. After a large or unknown caffeine exposure, seek urgent medical advice right away. A seizure, collapse, trouble breathing, severe confusion, or an irregular heartbeat needs emergency care. An intentional large ingestion is a mental health emergency.

What This Means for Your Family

  1. Start with curiosity, not a lecture. Ask what the drink does for your child. Is the goal staying awake, fitting in, sports, taste, or coping with poor sleep?
  2. List every source of caffeine. Include coffee, tea, soda, energy drinks, shots, pouches, chocolate, pre-workout powders, supplements, and over-the-counter medicines.
  3. Read the whole container. Check the serving size, the caffeine amount, guarana, and any other stimulants. Skip products when the amount is missing or unclear.
  4. Protect sleep. Notice whether later caffeine lines up with a longer time to fall asleep or a harder morning. Bring that pattern to the clinician.
  5. Share the list with the prescriber. This matters most with sleep trouble, anxiety, appetite loss, a pounding heart, high blood pressure, or viloxazine use.
  6. Skip caffeine “focus” products for ADHD. That includes caffeine pills, powders, pouches, energy shots, and caffeine-and-L-theanine “focus stacks.” Marketing is not a safety test. Independent testing has repeatedly found supplements with inaccurate labels or hidden stimulants.

If a daily user feels ill or gets headaches without caffeine, ask a clinician how to cut back safely. Do not turn this into punishment.
